I understand that information about me and my progress in the alcohol education program may be used for an annual report to the Department of State Health Services, research purposes (without identifying me), and may be shared with the court. I hereby authorize such use with the further understanding that this information will otherwise be held confidential and not released to other individuals or agencies for any reason without my signed consent. This consent may be revoked at any time, but is necessary for class participation.

No LATE Registration  you must sign-up on or before Thursday of the class start date!!!! NO STUDENT WILL BE ALLOWED TO ATTEND IF THEY ARE NOT signed-up early. Early registration is REQUIRED!

Deadline: Register ASAP, but no later than Thurs before class to avoid late registration fee. *If 3 students are not registered by deadline, class will be cancelled.

Class is 6 hours plus exit interview: see below, may vary!

Objective: MIP class  is a State approved  program for the participant in possession, purchase or  consumption of alcohol under the age of 21. The course can take a leading role in the effort to prevent intoxication and alcohol misuse.

Register in person during our office hours of Mon-Thurs from 12  noon-5:30 pm. Payment is due upon registration and must be paid by cash or check only.
